At HELP Carolina, we do not just connect people to services – we walk alongside them until they are stable. Every program below is built on a Housing First, trauma-informed, Social Determinants of Health approach, and every client relationship can continue for up to 24 months after housing is secured. That kind of follow-through is rare – and it is why 87% of the households we help remain stably housed. Because at HELP Carolina, no one walks this journey alone.

Referrals
Every person who comes to HELP Carolina – whether they walk through our door directly or arrive through a partner agency – starts with an immediate assessment of their most urgent needs.
We do not do generic hand-offs. If someone’s situation is better serviced by another agency or program, we make that connection ourselves, matched to what that person actually needs – not just the next available opening.
What sets us apart: We function as a trusted triage point for the entire tri-county Continuum of Care (NC-509) – not just an intake desk. Agencies refer clients to us specifically because they know we will asses honestly, even when that means connecting someone elsewhere.

Street Outreach
Our outreach team goes to where our unhoused neighbors are – not the other way around. We build relationships first, then bring case management, emergency essentials, and a pathway toward stability directly to neighbors who may not trust or be able to assess traditional services.
What sets us apart: We are mobile and community-embedded, led by people with deep roots in Gaston County – including team members with lived experience of homelessness themselves. That local trust means we reach neighbors other outreach models miss – and we stay in contact long enough for outreach to turn into real progress, not just a single encounter.

Emergency Shelter
When someone needs shelter now, we place our unhoused neighbors and their families in non-congregate settings – hotels and motels – rather than group shelter facilities. That means privacy, dignity, and safety from day one, paired immediately with the same intensive wraparound case management every HELP Carolina client receives.
What sets us apart: We are the only organization in the Gaston, Lincoln, and Cleveland tri-county area pairing non-congregate emergency shelter with long-term, wraparound case management. Clients are not just given a bed – they are given a plan, and a team that stays with them past the shelter stay.

Housing Navigation
Through our core Pathways to Stability program, we help clients locate housing and clear the barriers standing in their way – landlord engagement, benefits navigation, document readiness, and service planning tailored to each household. We use standardized assessment tools (VI-SPDAT/HART) to prioritize need and match clients with the right level of support from day one.
What sets us apart: Housing navigation at HELP Carolina is not a referral list – it is a hands-on advocacy with landlords and systems on the client’s behalf, grounded in a Social Determinants of Health approach that addresses income, transportation, healthcare access, and social isolation alongside the housing search itself.

Prevention
Homelessness is easiest to solve before it starts. Our Prevention & Diversion services work with households on the edge of losing housing – problem-solving in real time to help them avoid shelter entry altogether.
What sets us apart: We intervene early, before crisis turns into a shelter stay – addressing root causes through the same Social Determinants of Health lens we use in every other program.

Aftercare
For clients who move into permanent housing, our work is not finished at move-in. HELP Carolina provides up to 24 months of Aftercare – ongoing case management that helps households sustain stability long after most programs would have closed the file.
What sets us apart: Most providers serving those experiencing homelessness end their support at housing placement. Ours continues for up to two years afterward – which is a major reason our clients achieve an 87% stable housing retention rate, well above what short-term intervention models typically produce.

Coordinated Entry
As a partner in the tri-county homeless response system, HELP Carolina helps operate Coordinated Entry – a standardized process that uses shared assessment tools (VI-SPDAT/HART) to evaluate need and connect people to the right housing intervention, in the right order of priority, across Gaston, Lincoln, and Cleveland Counties.
What sets us apart: Coordinated Entry means no one falls through the cracks between agencies. Whether someone reaches us first or another partner organization first, the same standardized assessment ensures they are prioritized fairly and connected to the intervention that fits their needs.
